1) The adjudication website pages include the footer link to SFCC at the bottom of all pages. Can we drop that and put something similar to what's mocked-up in the screenshot (I'll try to include a screenshot)? Fixed-TP

2) For both clinical event and device event wizards (again, I'll try to add a screenshot to the wiki illustrating):

Fixed-TP

Could you also make a slight tweek to the clinical event data entry and the device event date entry worksheet templates?

Could you add a header title of "STEP" to the left column listing all the adjudication screen steps?

Could you revise the right header from "(Step 6 of 33) / Stroke evidence?" to a single line [using this screen as an example] "6: Stroke Evidence?"

- should be reconciled with data from the "Outcome Report" CRF; discordances should be flagged for Jim or Lewis to review (if site says they died and adjudicator says they didn't, or vice-versa). [TP: Please discuss this reconciliation requirement with Mario] [MJP - Discussed with Mario and Jim and Lewis - Mario will flag cases with discrepancies for review by Jim or Lewis, who will resolve the discrepancy before the case is sent to other adjudicators and/or finalized] - Jim, let me know if this issue has been worked out.

(sorry) resolution seems fine to me [jd]
